As part of the existing comprehensive quality monitoring process of local health care services, the CCG is expanding the opportunities for people to feedback on their experience of health care locally.
We therefore welcome your feedback on the health services in south Warwickshire commissioned by South Warwickshire CCG.
We would like you to tell us about your experiences, good or bad, of local healthcare services. You can also give suggestions as to how improvements could be made. As an alternative to submitting the online form, you can email your feedback directly to email@example.com .
When you submit your feedback, please do not include any information that identifies you personally. For example, do not include your name, address, date of birth, hospital number, etc. within the body of the document. This personal information is protected and remains confidential. We only require a summary of your overall experience.
We are unable to respond individually to your feedback because it is sought solely to gather information which we will collate, analyse and theme and use the findings to continually improve and plan our local services.
Please note, however, the CCG Feedback System will not to be used to deal with individual complaints or concerns. If you wish to complain about local health services please click here.
Form submissions are stored on the server for 28 days and are encrypted and secured with a private key known only by the GP Practice.
