Subject Access Requests

You can find out if we hold any personal information by making a ‘subject access request’ under the Data Protection Act 2018. If we do hold information about you we will:

·         Give you a description of it;

·         Tell you why we are holding it;

·         Tell you who it could be disclosed to; and

·         Let you have a copy of the information in an intelligible form.

To make a request please contact the CCG by emailing or calling 01926 353729. You will need to provide us with your:

  • Full Name;
  • Address;
  • Date of Birth;
  • Preferred contact details;
  • The type of information the individual is looking for, for example an individual may have made a complaint to your organisation and now require all information that is filed related to the complaint.

In some cases it may be necessary for us to ask you to provide more information before we can respond to your request.
